Ask for the image files and the written radiology report needed for your next appointment. Confirm that the receiving clinic can open them.
Choose the records
- Ask the next clinician which examinations and dates are needed. Include earlier studies if requested for comparison.
- Request the written report separately from image files. A photograph of a screen may leave out information needed for clinical review.
Check the format
- Ask what format the hospital provides and whether a viewer is needed. Confirm compatibility with the receiving clinic before you travel.
- Check your name, examination date and body area on the records. Report mismatches to the issuing hospital.
Arrange a secure handoff
- Confirm how to collect the files, any fee and any access expiry. Ask the hospital about replacements if files will not open.
- Share through a method agreed with the receiving clinic. Do not post identifiable records publicly; ask a clinician to explain the report.
